The top three reasons doors fail on inspection

Curious how your findings compare to mine. At the sites I am currently surveying, the top three fail causes are: 1. Gaps too wide (over 4mm at head or jambs) 2. Missing or damaged intumescent strips 3. Self-closers not fully closing against the latch What is topping your failure lists? And which ones are most frustrating to explain to building owners?
